Need a resource to help an elderly stroke victim in NW Phoenix

So, I got a call from a friend in Tucson last night. She has a friend who lives alone in NW Phoenix who recently had a stroke and a heart attack. She  was sent home from the hospital a month or so ago and has been struggling.

Mt friend isn't familiar with Phoenix so she called me asking if I knew of any resources. The only one that comes to mind is Meals on Wheels which would probably be a godsend  but I think she could probably also benefit from someone checking in on her daily. Any recommendations?

Reach out to the Foothills Caring Corps. Not sure they are active on that side of town but they might refer you to another outfit if not
